You must, at the time of completing this form, advise Haileybury of any matters that pertain to the educational, physical or psychological development of your child, as this information is of great importance in permitting us to assess their needs. Failure to provide this information constitutes grounds for Haileybury to terminate the enrolment at our discretion, where we reasonably determine that it is not practical for us to continue the enrolment.
Non-provision of such information or provision of inaccurate information will be treated as breach of these terms and conditions of enrolment and may result in termination of enrolment at Haileybury’s discretion, where we reasonably determine that it is not practical for us to continue the enrolment.
